The permission model, in detail

Permissions are granted per doctor and per area of work, so an assistant helping two doctors holds two separate, visible sets of access.

Every grant, and every use of access, is recorded. In security terms: access is deny-by-default, scoped, auditable and revocable with immediate effect.
